Introduction:

Aceclofenac, Paracetamol and Serratiopeptidase tablets (Lace-SP) are a blend of three molecules that are used to treat pain, fever, and inflammation. 

Aceclofenac is a nonsteroidal anti-inflammatory drug (NSAID) class drug. It works by blocking the production of certain hormone that is responsible for causing pain and inflammation, namely prostaglandins.

Paracetamol is a well-known analgesic and antipyretic drug that helps to get relief from pain and fever. Serratiopeptidase is an enzyme that is used to treat inflammation and swelling. The synergistic effects of three molecules make this tablet a strong pain reliever medicine.

Serratiopeptidase–

Serratiopeptidase (Serratia E-15 protease, also known as serralysin, serrapeptase, serratiapeptase, Serratia peptidase, serratiopeptidase, or serratiopeptidase), is a proteolytic enzyme (protease) produced by non-pathogenic enterobacteria Serratia sp. E-15, now known as Serratia marcescens ATCC 21074. This microorganism was originally isolated in the late 1960s from silkworm bombyx Mori L. (intestine), Serratiopeptidase is present in the silkworm intestine and allows the emerging moth to dissolve its cocoon. Serratiopeptidase is produced by purification from the culture of Serratia E-15 bacteria. It is a member of the Peptidase M10B (Matrixin) family.

Aceclofenac-

Aceclofenac is a nonsteroidal anti-inflammatory drug (NSAID) analog of diclofenac. It is used for the relief of pain and inflammation in rheumatoid arthritis, osteoarthritis, and ankylosing spondylitis.

Aceclofenac was patented in 1983 and approved for medical use in 1992.

CHEMISTRY OF ACECLOFENAC –

Aceclofenac (C16H13Cl2NO4), chemically [(2-{2, 6-chlorophenyl) amino} phenylacetooxyacetic acid], is a crystalline powder with a molecular weight of 354.19. It is practically insoluble in water with good permeability. It is metabolized in human hepatocytes and human microsomes to form [2-(2′,6′-dichloro-4′-hydroxy- phenylamino) phenyl] acetoxyacetic acid as the major metabolite.

Paracetamol:

Paracetamol, also known as acetaminophen, is a medication used to treat pain and fever. It is typically used for mild to moderate pain relief. Evidence is mixed for its use to relieve fever in children. It is often sold in combination with other medications, such as in many cold medications. Paracetamol is also used for severe pain, such as cancer pain and pain after surgery, in combination with opioid pain medication. It is typically used either by mouth or rectally but is also available by injection into a vein. Effects last between two and four hours

Paracetamol is generally safe at recommended doses. The recommended maximum daily dose for an adult is three to four grams. Higher doses may lead to toxicity, including liver failure. Serious skin rashes may rarely occur. It appears to be safe during pregnancy and when breastfeeding. In those with liver disease, it may still be used, but in lower doses. It is classified as a mild analgesic. It does not have significant anti-inflammatory activity. How it works is not entirely clear.

Paracetamol was first made in 1877. It is the most commonly used medication for pain and fever in both the United States and Europe. It is on the World Health Organization’s List of Essential Medicines. Paracetamol is available as a generic medication, with brand names including Tylenol and Panadol among others. The wholesale price in the developing world is less than US$0.01 per dose. In the United States, it costs about US$0.04 per dose. In 2017, it was the 25th-most commonly prescribed medication in the United States, with more than 24 million prescriptions.

  • Women in pregnancy and lactation should not take.
  • Since this combination has slightly sedative properties do not take it  prior to driving
  • Do not co-administer alcohol
  • In Gastric Ulcers should not be used.
  • Should not be used for asthma

Controversies associated with this combination: 

The Health Department of the Indian Government issued a notice to all States and UTI's Drug Controllers to withdraw the permission to manufacture 294 FDCs that were opposed to use by several consumer associations. The first time, the issue was raised in 2007, In December 2018, CDSCO issued notices to all state drug controllers to cancel permissions of above said 294 FDCs. However, CDSCO allowed 49 FDCs from 294 to manufacture them after submitting sufficient clinical trial data in support of these items. Aceclofenac, Paracetamol, and Serratiopeptidase tablet are 3 molecules of FDC that was also needed to submit clinical trial data that support the effectiveness of this combination over two molecules (Serratiopeptidase and Paracetamol) FDC.
